Prepare for Your Visit

Thank you for entrusting Dr. Wang with your care. The following will answer many of the questions that you may have regarding your visit with Dr. Wang. Please read through the information below before you visit. If you are a new patient, please complete the New Patient Questionnaire and bring it with you on the day of your appointment.

New Patient Information

Our goal is to provide you with the quality care that you deserve. We do our best to streamline the process and be respectful of your time. You can help us by filling out the New Patient Questionnaire and other important items listed below. Please arrive early to allow time for registration and any additional X-rays that may be requested by Dr. Wang prior to your visit.

What to bring to your appointment:

  • Previous Images (X-ray, MRI, CT scan, etc) that have been obtained at an outside facility. Please make sure to bring the CD(s).

  • Operative Reports and Medical Records related to your affected extremity

  • Insurance Card

  • Referrals/Authorizations if your plan requires

  • Copayment – Exact cash, check, or credit card (Amex, Visa, MC)
